The drama unfolds when Dr. Liz Shaw raises concerns over Harper’s latest ultrasound results, prompting her to call in cardio surgeon Levi for a second opinion. Together, they break devastating news to Harper and Tan—their baby has developed a cystic adenomatoid malformation (CCAM), a benign lung lesion that, if left untreated, could put intense pressure on the baby’s organs, even leading to heart failure.
Further scans reveal that the cyst is larger than expected, forcing Harper and Tan to make an impossible decision. They can either monitor the cyst and hope it doesn’t worsen—risking both Harper and the baby’s lives—or opt for high-risk in-utero surgery. Though hesitant at first, Harper ultimately agrees to the surgery after encouragement from Tan and Levi.
As Levi prepares for the delicate operation, he confesses to girlfriend MacKenzie that the pressure of operating on a close friend’s child is overwhelming. When the moment arrives, the surgery doesn’t go as planned. Levi struggles to remove the cyst, forcing him to use a laser scalpel, a method that demands absolute precision. Meanwhile, Harper’s blood pressure spikes dangerously, putting her at serious risk. Dr. Karana, the obstetric surgeon assisting, warns that if her condition worsens, they’ll have no choice but to deliver the baby immediately.
Back in the hospital ward, Tan grows frustrated, desperate for updates. He lashes out at Dana, who is equally anxious but unable to provide any news. Tensions rise, but Dana ultimately chooses to walk away before emotions boil over.
Finally, after a nerve-wracking operation, Levi successfully removes the cyst, and both Harper and her baby survive the procedure. But just as relief settles in, a new crisis emerges—Harper wakes up the next morning with intense stomach cramps. As the machines start beeping, she turns to Tan in fear and says the words no one wants to hear: “I think I’m having a contraction.”
Dr. Shaw acts fast, administering an injection to slow the contractions, but the danger isn’t over. Outside, Tan pleads with Levi for answers—if baby Archie is born at just 26 weeks, what are his chances of survival? Levi doesn’t sugarcoat it—while premature babies can survive, the risks of complications in neonatal intensive care are extremely high.
