Behind the Scenes! Casualty Star Michael Stevenson Takes Digital Spy on an Exclusive Tour of the Set

Casualty confirms early episode releases

Ever wondered what the Casualty emergency department looks like when the cameras stop rolling? Fans were recently treated to a rare behind-the-scenes look as series regular Michael Stevenson, who plays paramedic Iain Dean, took Digital Spy on an exclusive tour of the beloved medical drama’s set — and it’s as detailed and immersive as the show itself.

🚑 Welcome to Holby ED – But With a Twist!

From life-or-death scenes to high-stakes trauma cases, Casualty’s ED looks intense on screen. But as Stevenson cheerfully guides viewers through the maze-like halls of the set, it quickly becomes clear just how much work goes into creating the illusion of a functioning hospital.

“You wouldn’t believe how much of what you see is actually just clever design,” he explains. “It looks like a real emergency ward — until you open a cupboard and find it’s just a flat wall!”

The set, located at Roath Lock Studios in Cardiff, is a purpose-built replica of a modern NHS hospital, complete with treatment bays, consultation rooms, and even an ambulance bay.

🎬 Filming Tricks Revealed

Stevenson shows off the “crash bay”, one of the most commonly used spaces in high-intensity scenes. While it looks real on screen, viewers learn it’s all about angles and lighting. The rooms are actually modular, allowing crews to move walls, adjust camera setups, and shoot from multiple perspectives.

“What looks like a bustling ward is often just a few extras walking past the same corridor five times,” he laughs.

He also shares how the actors simulate medical procedures, including the use of realistic dummies, prop syringes, and expert on-set medical advisors to keep things authentic.

💡 Surprising Set Secrets

During the tour, Stevenson points out some surprising details, including:

  • The nurses’ station, where many pivotal conversations take place, is stocked with real hospital documents and props to maintain realism — even though they’re rarely seen in detail.

  • Iain’s locker in the paramedic bay has subtle personal touches, including a photograph and items that reflect his backstory.

  • The green screen ambulance drive-in allows for “night” and “storm” scenes to be created entirely indoors.

👨‍⚕️ Michael’s Favourite Spot on Set?

“It has to be the paramedic station,” Stevenson says without hesitation. “It’s where Iain feels most at home, and it’s where I’ve spent the most time over the years. There’s a lot of character history in that room.”

He also revealed a few off-screen antics, including pranks with fellow cast members and bloopers that happen during intense scenes — especially involving fake blood and collapsing stretchers!

🧠 A Tribute to Real NHS Heroes

Throughout the tour, Stevenson repeatedly emphasises the respect the cast and crew have for NHS staff. Every part of the set is designed with real-world accuracy, thanks to consultations with healthcare professionals.

“We may be actors, but we take portraying their work seriously. This show is a tribute to them.”
